  • Associates degree in Human Services field from an accredited university preferred

Minimum Work Experience:

  • Prefer two years experience working with families and/or children

Minimum Job Requirements:

  • Must be 21 years of age
  • Must pass drug screen, MVR, KBI, Child Abuse and Neglect Central Registry Clearance check and Adult Registry check
  • FBI Fingerprint
  • Lifting requirements of 30-50 lbs.
  • Must have valid drivers license, acceptable driving record and reliable transportation

Essential Functions:

  • Develops an understanding of the child or youths needs for environment and caregiver, wants in a placement, personal strengths and coping skills, cultural beliefs and creates and individualized referral to make the most appropriate match for the child or youth transitioning from a higher level of care (PRTF & QRTP)
  • Updates the individualized referral timely and it is comprehensive and representative of the child presently
  • Participates in all discharge meetings; when unavailable locates alternate person to participate
  • Communicates regularly with the case team for the child/youth as well as other stakeholders for the child/youth
  • Assists data personnel with appropriate paperwork related to placement
  • Completes all documentation necessary to ensure that appropriate placements are located for clients. This includes CMS and all other documentation platforms utilized
  • Maintains contact with all child placement agencies and foster parents to develop and maintain positive rapport
  • Establish relationships and resource list for homes specializing in care for high needs children/youth
  • Adapts to a flexible work schedule.
  • Completes accurate and timely documentation as assigned
  • Participates in Placement Coordinator holiday on-call rotation
  • Implements Saint Francis Ministries policies and follows directives as required. Follows and adheres to all pertinent Saint Francis Ministries Standard Operating Procedures (SOPs), rules, personnel policies and procedures; related accreditation and licensure standards; and federal, state and local rules, statutes, regulations, and all term of the contract between Saint Francis Ministries and DCF
  • Reports unusual incidents through appropriate Risk Management, clinical and safety channels.
  • Ensures clients rights are protected
  • Is knowledgeable of and follows all safety procedures

Required Skills, Knowledge, and Abilities:

  • Mandated Reporter
  • Demonstrate effective written and verbal communication skills.
  • Basic understanding of family centered practice, child development, family systems, and treatment.
  • Understands child and adolescent therapeutic techniques and how this impacts the client.
  • Sensitive to the needs of youth and their families.
  • Understanding of the child welfare and state foster care/adoption system.
  • Understanding of mental health state systems and Medicaid systems.
  • Must be a team player.
  • Flexible to changing situations, staff and youth.
  • Able to solve problems, but also able and willing to seek assistance when necessary.
  • Independent discretion/decision making within the scope and responsibility of the position.
  • Ability to think critically and objectively as well as evaluate needs and progress
