- Associates degree in Human Services field from an accredited university preferred
Minimum Work Experience:
- Prefer two years experience working with families and/or children
Minimum Job Requirements:
- Must be 21 years of age
- Must pass drug screen, MVR, KBI, Child Abuse and Neglect Central Registry Clearance check and Adult Registry check
- FBI Fingerprint
- Lifting requirements of 30-50 lbs.
- Must have valid drivers license, acceptable driving record and reliable transportation
Essential Functions:
- Develops an understanding of the child or youths needs for environment and caregiver, wants in a placement, personal strengths and coping skills, cultural beliefs and creates and individualized referral to make the most appropriate match for the child or youth transitioning from a higher level of care (PRTF & QRTP)
- Updates the individualized referral timely and it is comprehensive and representative of the child presently
- Participates in all discharge meetings; when unavailable locates alternate person to participate
- Communicates regularly with the case team for the child/youth as well as other stakeholders for the child/youth
- Assists data personnel with appropriate paperwork related to placement
- Completes all documentation necessary to ensure that appropriate placements are located for clients. This includes CMS and all other documentation platforms utilized
- Maintains contact with all child placement agencies and foster parents to develop and maintain positive rapport
- Establish relationships and resource list for homes specializing in care for high needs children/youth
- Adapts to a flexible work schedule.
- Completes accurate and timely documentation as assigned
- Participates in Placement Coordinator holiday on-call rotation
- Implements Saint Francis Ministries policies and follows directives as required. Follows and adheres to all pertinent Saint Francis Ministries Standard Operating Procedures (SOPs), rules, personnel policies and procedures; related accreditation and licensure standards; and federal, state and local rules, statutes, regulations, and all term of the contract between Saint Francis Ministries and DCF
- Reports unusual incidents through appropriate Risk Management, clinical and safety channels.
- Ensures clients rights are protected
- Is knowledgeable of and follows all safety procedures
Required Skills, Knowledge, and Abilities:
- Mandated Reporter
- Demonstrate effective written and verbal communication skills.
- Basic understanding of family centered practice, child development, family systems, and treatment.
- Understands child and adolescent therapeutic techniques and how this impacts the client.
- Sensitive to the needs of youth and their families.
- Understanding of the child welfare and state foster care/adoption system.
- Understanding of mental health state systems and Medicaid systems.
- Must be a team player.
- Flexible to changing situations, staff and youth.
- Able to solve problems, but also able and willing to seek assistance when necessary.
