    Position Summary

    This is a professional OPS position with the responsibility of a QSI Assessor who conducts evidence-based assessments for the Agency for Persons with Disabilities using the Questionnaire for Situational Information for all APD eligible persons.

    The Work You Will Do

  • Successfully complete assessments using the QSI, which includes reviewing information gathered from the person’s records; conducting face-to-face interviews with the person with a developmental disability and the person’s guardian and family; and interviews the person’s caregivers, health care personnel, waiver/pre-enrollment support coordinator, or service providers as appropriate, verifying demographic information; and assessment results with the waiver/pre-enrollment support coordinator; as well as entering the results into the QSI online application.

  • Review the person’s central record/ iConnect record including recent assessments, progress notes from medical records, school records, previous support plans, and other collateral sources, as appropriate.

  • Refer persons and family members to their Waiver or Pre-enrollment Support Coordinator for provider information and available resources in the community, based on needs identified during the administration of the QSI.

  • Schedule meetings and follow-up telephone sessions to conduct interviews with the person eligible for services from APD, family members, guardians, waiver/pre-enrollment support coordinator, and providers, and support staff if needed, to complete a comprehensive review of the person’s needs for completion of the QSI.

  • Communicate a person’s needs for follow-up by the Regional Medical Case Manager or Regional Behavior Analyst following completion of the QSI.

  • Verify that all demographic information is correct for the person being assessed. If the information requires updating, alert the person’s waiver support coordinator/pre-enrollment coordinator of the corrections required in iConnect to assure accuracy.

  • Attend meetings, complete training, and annually demonstrate inter-rater reliability on the QSI for recertification as an QSI Assessor.

  • Perform other duties as assigned, including administrative tasks.

    Minimum Qualifications

  • Valid Driver’s License or other efficient means of transportation to travel for work purposes.

  • Must have a bachelor’s degree from a college or university and a minimum of four (4) years professional experience working with persons with developmental disabilities.

  • Must demonstrate excellent communication skills used in conducting person-centered interviews and sufficient computer skills to access and use a web-based assessment system to obtain demographic and other details specific to locating persons and to record results of assessments.

  • Daily travel required to conduct assessments.

    Knowledge, Skills, And Abilities

    Knowledge of:

  • The QSI protocol: its purpose, ability to score and interpret QSI items correctly.

  • The person-centered process used in conducting interviews for the QSI.

  • APD processes and procedures, including i-Budget/ Waiver and pre-enrollment .

    Ability to:

  • Use computer-based systems, including the APD intranet: iConnect, SharePoint, Microsoft Office, Microsoft Teams, and Excel spreadsheets.

  • Communicate effectively in face-to-face, electronic, and written formats.

  • Establish and maintain effective working relationships with persons, families, providers, and staff within the region and statewide .

  • Prepare reports, and manage, organize, analyze, and monitor data related to the QSI.

  • Plan, organize and coordinate work assignments.

  • Meet weekly QSI assessment quotas.

  • Effectively communicate, respectfully conversing with others to achieve a desired outcome.

  • Establish and maintain effective working relationships with others.

  • Travel for work purposes.

  • Administer APD approved functional needs assessments.

    Direct Deposit Program

    As a condition of employment, a person appointed to a position in the State Personnel System is required to participate in the Direct Deposit Program. Rather than receiving a paper paycheck, your funds will be deposited directly into your account at your financial institution. This will be accomplished by Electronic Funds Transfer. Banks, savings and loan associations, and credit unions are eligible to accept such deposits. Retirement funds are also required to be in the Direct Deposit Program.

    Background Screening Requirement

    It is the policy of the Florida Agency for Persons with Disabilities that applicants for employment undergo Level 2 employment screening in accordance with the requirements of Chapter 435, Florida Statutes, as a condition of employment or being permitted to serve as a volunteer.

    No applicant for a designated position will be employed or permitted to volunteer until the Level 2 screening results are received, reviewed, and approved by the Agency.

Level 2 background screening shall include, but not be limited to, fingerprinting for Statewide criminal and juvenile records checks through the Florida Department of Law Enforcement and Federal criminal records checks through the Federal Bureau of Investigation and may include local criminal records checks through local law enforcement agencies.

Pursuant to S. 110.1127(1), Florida Statutes, this position requires a background investigation, including fingerprinting.
