The role of the Atrial Fibrillation Clinic Coordinator is to be responsible for the coordination of clinical services at the population level for patients of the Atrial Fibrillation Clinic. The position will manage the AFC population and be tasked with growing that population appropriately within our footprint. They will coordinate with procedural and clinic schedulers, nursing staff, AFC clinicians (PAs and NPs), and supervising electrophysiologists to ensure that each patient's established AF treatment plan is fulfilled.

- Facilitate care coordination from acute to ambulatory settings. - Collaborate with Physicians, Case Managers, and other healthcare team members to ensure smooth coordination of care and follow-up appointments. - Serve as a resource for care coordination, providing guidance for medically and/or psychosocially complex cases. - Maintain current knowledge of available community resources, post-acute care, and ambulatory options. - Implement NHFDNH safety behaviours and high reliability strategies to enhance patient safety and experience.

Demonstrated ability to deliver exceptional patient experiences. - Strong collaboration skills with a multidisciplinary healthcare team. - In-depth knowledge of community resources, post-acute care, and ambulatory options. - Commitment to Novant Health’s values of respect, compassion, inclusion, empowerment, inspiration, and courage.
C ardiology background, preferably electrophysiology (cardiac devices).
-Create and manage a database of AF Clinic patients for quality assurance and to guide clinic directions
-Assist patients in navigating the healthcare system, ensuring access to necessary resources, support services, and community programs
-Arrange AF talks with primary care groups
Pursue opportunities to increase clinic exposure
Education
4 Year / Bachelors Degree Preferred
Relevant fields of study include Nursing, Social Work, or Healthcare Administration.
Licensure/Certification: Active RN licensure Required
