We are hiring a Clinical Care Manager for the Refugee Program in Worcester. This role coordinates care for refugee patients at high risk for health issues, focusing on patient education and self-management to improve outcomes. The Clinical Care Manager ensures care meets national, state, and health center standards. Working within a team-based approach, this role involves coordinating patient care, supporting self-management, using evidence-based methods, participating in patient-focused projects, and employing effective communication with patients.

Essential Functions:
Assists the Refugee provider in coordinating culturally sensitive care for refugee patients, covering acute and chronic illness management, immunizations, lifestyle education, and preventive care across settings.
Completes the nursing process, collaborates with a multidisciplinary team, performs treatments, and provides physical, emotional, and educational support.
Works with the clinical team to manage patient care, addressing health, financial, and psychosocial barriers.
Communicates patient needs and care plans with the PCP, team, and family. Supports refugees in achieving self-sufficiency and involves families in care decisions.
Utilizes community resources, assists in action planning for self-management, and uses behavioral strategies to promote healthy behaviors and independence in healthcare navigation.
Required Qualifications:
Registered Nurse License in MA
Current BLS CPR certification
Excellent interpersonal skills are necessary
Excellent Verbal and Written Communication Skills
Experience working with multicultural, multiethnic populations.
